Nonimmigrant Visa Forms

Every applicant for a nonimmigrant visa (NIV), including children, must present a separate, completed and signed Electronic Visa Application Form DS-156 (3 pages) at the time of the interview at the U.S. Embassy. Handwritten applications are no longer accepted. Applicants are reminded to come unaccompanied to the Embassy. Due to limited space, only visa applicants will be allowed into the Embassy, all other family members, friends or colleagues should remain at home.

Supplemental Application -- All male nonimmigrant visa applicants between the ages of 16-45, regardless of nationality or other factors, must now complete a supplemental application form Form DS-157 [PDF - 128kb]which helps inform the consular officer's judgment about visa eligibility. Consular officers have the authority to require anyone from any country to complete visa forms if they think it's warranted.


Student and Exchange Visitor Visas - All student and exchange visitors (F1, M1 and J1 visas), regardless of nationality or other factors, must also complete a supplemental application form - DS-158 [PDF - 128kb] which helps inform the consular officer's judgment about visa eligibility.  Applicants must also get an authorization form from their sponsoring institution in the U.S. Before a visa can be issued, the sponsoring institution must authenticate the applicant by opening and maintaining an electronic file on a Web-based data management system managed by the Department of Homeland Security, called Student and Exchange Visitor Information System (SEVIS).

If someone other than the applicant completes Form DS-156, the person completing the form must sign his/her name at the bottom of item #40 on Form DS-156. Applicants are responsible for the accuracy of the information provided in the application forms.

Important: After completing the form, you must press the "CONTINUE" button at the bottom of the form, then print the form and bring all THREE PAGES to the U.S. Embassy on the day of your interview.
